All Students

• Immunization Record Form

To access this form:

1. Log into my.MCAD.edu
2. Click on "Forms-Student Life" on the left side of the page.
3. Click on "Immunization Record"
4. Hard copy of the immunization form must be submitted if the student is under 18 years old or is consciously objecting to the immunizations.
5. Please do not email/mail full immunization reports. Complete the online form with the appropriate immunization dates.
6. Students who attended a Minnesota high school will need to log into the form, but will not need to submit the dates (See the form for more information).
